For an appointment at the WHS Spay & Neuter Clinic, you must complete ALL fields. If you have more than 1 pet, you will need to fill out the form for each pet. We will make every effort to schedule dates together for multiple-animal households. Your appointment slot will be confirmed by a follow up email within two business days, so make sure to check your SPAM filter if you have not heard from us within that timeframe.
